1) Change the TOCs of Part 1 and Part 4.

2) The change to Part 4, A.4.1 is unnecessary, 
since this simple type is already there.

3) Remove the following defition from the RNG schema shown in B.4.1
DrawingML - Main.

a_ST_PercentageDecimal = xsd:int
