This is just a draft of DRs, but I thought that this draft might help.

Defect-A

Part 2

The RNG schema for OPC disallows @xml:lang in dc:creator and other
elements, although the XSD schema does allow it.  Fix the RNG schema.

Defect-B

Part 2

OPC does not allow Japanese users to specify dc:creator twice (once in
Kanji and once in Kana).  This is a serious problem for the conversion
from OOXML to EPUB and ODF.  Allow more than one occurrence of dc:creator, 
and do the same thing for other Dublin Core elements.

Defect-C

17.6.5  docGrid (Document Grid), Part 1

Three firgures in this subclause are completely unreadable.
Replace them with readable figures.

Defect-D

17.6.11 pgMar (Page Margins) , Part 1

The relationship between gutter, text margins, footers, headers,
and pages is unclear.

Add diagram for depicting gutter, text margins, footers, headers,
and pages.

Defect-E

The XSD and RNG schemas in 29500-4 do not allow v:textbox of VML to
have unqualified <div> elements as children, although those in
Ecma-376 (1st Ed) do.  However, existing docx documents contain such
<div> elements for representing HTML fragements.
