Proposed appendix for page layout (including Kihon Hanmen)

Dear colleagues,


In Barcelona, we discussed this document and agreed that
BIDI has to be covered.

In multi-column BIDI documents, the first column is the right column
rather than the left column.  This is the biggest difference.

I have updated the document accordingly.  Here goes.

Although wordsmithing is certainly required, I hope to adopt
this document and close  DR 11-0008 thru -0011 in the
next F2F.
