DR 13-0005, "SML: Clarify descriptions of header and footer usage": Proposed Resolution

Attached is my proposed response to China's DR requesting clarification on
SML headers and footers. The spec is indeed missing more than a little
information necessary to deal with these things.

Although the response document is 14 pages long, the changes in almost all
of the affected subclauses are rather small. The bulk of the edits result
from moving much of the text from 18.3.1.39 to 18.3.1.46, and the
expansion/rewrite of that text. Once these changes are integrated into the
main standard, the page count should increase by only a page or two.
