Implementing MCE in XSLT2

Dear colleagues,

Now, my implementation of MCE in XSLT2 (see the quoted e-mail)
covers all examples in the current draft.  The examples are available at:

mceTest.html in this directory provides a summary of test cases.
This html file is generated by mspec-humanReadableHtml.xsl, which was
originally written by Inigo.
